Hope Community Support Program

Providing adult rehabilitative mental health services in a community setting


The Hope Community Support Program supports adults living with mental illness as they cope with the challenges of daily living, assisting each person with achieving and maintaining his or her highest level of self sufficiency. The outreach center is located at 157 Roosevelt Road, Suite 300, and is open Monday through Thursday, 9:00 a.m. to 3:00 p.m., and Friday, 9:00 a.m. to noon. Individual outreach services are available to eligible clients by appointment.

A comprehensive range of services are available to individuals of all faiths and beliefs, depending upon the eligibility of the individual.

Traditional Community Support Program Services

Adults living with "serious and persistent mental illness" who DO NOT receive Medical Assistance may qualify for our traditional services.

Adult Rehabilitative Mental Health Services (ARMHS)

Adults living with mental illness who receive Medical Assistance may qualify for the Adult Rehabilitative Mental Health Services (ARMHS) portion of Hope Community Support Program=s services. These services (link) enable the client to develop and enhance psychiatric stability, social competencies, personal and emotional adjustment, and independent living and community skills. Referral information

Hope Community Support Program accepts referrals from a variety of sources, including health care providers, mental health professionals, families, and individuals seeking service.
